William P. Smith, RPh., M.S., President, is an experienced pharmacist and executive with a broad knowledge of quality improvement, management strategies, medical-legal consulting, marketing, and clinical program development buttressed by more than 10 years of significant hands-on consulting experience in hospital, managed care, and retail pharmacy settings with an emphasis on drug safety, quality healthcare delivery and related industry trends.






Sangeetha Subash, Data Analyst
Sangeetha has significant hands-on experience with IT Development.  She has experience using IBM’s SPSS Modeler, Cognos® Business Intelligence, and Microsoft SQL.  Key responsibilities include data acquisition and validation, data analysis and reporting using state-of-the-art analytic and reporting tools including  Key focus includes error trend analysis, data mining, error forecasting using predictive analytics, error research using comparative- and macro-analytics.


Dina Smith, CPhT., Senior Analyst
Dina is a nationally certified pharmacy technician responsible for system-based medication error investigation, interpretation and evaluation. Her expertise includes the review and qualification of Retail Pharmacy Unit Customer Relation’s Complaints specific to Quality Assurance and Safety issues. Dina is instrumental in the research and documentation of root cause for medication error events.


Martin Abramson, RPh., M.S., Safety Investigator
Marty is a Registered Pharmacist with over 30 years experience in the Veterans Affairs Healthcare System serving as the Director of Pharmacy for the Boston VA Healthcare System.  He has served on several national committees involving hospital Information system design, national formulary development, medication and adverse event reporting and patient safety.  Marty’s responsibilities at The PSO Advisory include expertise in Root Cause and Contributing Factors Analysis, Failure Mode Effect Analysis (FMEA) using FMEA tool designed by the Veteran’s Administration.  


Susan Mills, R.N.,  Manager Patient Safety
Sue is an experienced healthcare manager with over 30 years experience as a Registered Nurse.  Her responsibilities at The PSO Advisory include Clinic Account Management as well as directing safety analysis of quality and safety related events associated with retail pharmacy ambulatory care clinics.
